PMHA is happy again to partner with Archway Health & Sports Services to provide the mandatory Concussion and Baseline physical testing for the 2017-2018 Season.
**PLEASE NOTE CONCUSSION PHYSICAL TESTING IS
MANDATORY FOR
ALL ATOM THROUGH MIDGET
AND THOSE PLAYERS AGE 13 AND UP WILL BE GIVEN INFORMATION TO COMPLETE
THE COMPUTER PORTION OF TESTING AT HOME
.
PLAYERS WILL NOT BE ALLOWED TO PARTICIPATE IN TRYOUTS,
PRACTICES, EXHIBITION GAMES OR PRACTICES UNTIL FULL (BOTH PHYSICAL AND
COMPUTER IF REQUIRED) TESTING IS COMPLETED - NO EXCEPTIONS.**
This sign-up is for the physical testing component of the
testing (balance, grip, reaction, etc...) only.
Please note the sign-up will lock down six (6) days prior to each
date
to allow Archway to prepare for each session. This testing will take
approximately 30-60 minutes depending on each player. This
is required for all Atom to Midget aged this season. A data collection form
will need to be completed prior to the testing being completed.
The cost is for players 12 years old and under is $45 and for players
aged 13 and up is $55

. All physical testing will take place at
Archway Health & Sports Services, 4130 Glenview Road in Petrolia
(in the new Family Health Team Plaza).
