for Conversion of a renewal/upgrade of Certification from another Branch/Member

If you have submitted a request
for Renewal of your NCCP Certification through the Continuing Education Program,

If you are
submitting a Referee Crossover Registration, please ensure you submit your
Registration, along with Payment, and Police Record Check (if new to the OMHA
and a PRC is not on file with the originating Member Partner), or the Criminal
Offence Declaration.
