I agree that if selected I will, (i) Upgrade my coaching certificates where required and as requested by PMHA, (ii) Attend all applicable meetings, (iii) Abide by the PMHA Constitution and (iv) Comply with all rules and regulations of PMHA.
(Signature required if granted a coaching position)

All applications will be reviewed by the PMHA Executive.
Interviews will be conducted and ALL applicants will be contacted
even if they are not successful in being selected for a coaching position.
Successful applicants will be contacted and then posted on the website.
