ATTENTION ALL PLAYERS AND PARENTS CONCUSSION TESTING FOR 2021 – 2022 SEASON
Hello Parents & Players,
As we work to get back to normal, we continue to try to make sure we are covering off everything that needs to be done to keep all players safe while they play. In saying this we will be moving forward with our mandatory concussion testing for all players age U9 (formerly Atom) to U18 (formerly Midget)during the 2021-2022 season. Signing up for testing will be done a little differently this year.
Players or parents can call directly to Archway Sport & Health – Petrolia @ 226-738-0680 and speak to an Archway representative to sign up for one of the time slots being offered during out 3 November Concussion
Clinics.
The clinics will take place on November 17, 18, & 19th from 9AM – 4PM. You may also call and
set up a private appointment outside of these clinic date & times. We are starting with the older age
groups (U15 & U18) who will be exposed to highest chance for concussions with the inclusion of body
contact. However please feel free if you have a younger player to call and book a appointment to stay
ahead of the game. The cost of the testing for players this year will follow the price structure below.
Parents should check with their benefit plans as many will cover this type of testing for children. For
players in the U15 & U18 age groups the concussion testing is a 2-part process. Players will require an
appointment to complete the Baseline portion of the testing and then they will also be required to go
online and complete the computer portion of the testing. It is very important that BOTH parts of the
testing are completed to give Archway a full assessment of the player. For player U9 to U13 the Baseline
Testing portion is required.

U15 & U18 - $65.00
U9 thru U13 - $55.00
We are looking to have all U15 & U18 players completed by Dec 5 th , 2020. This gives everyone approx. 3
weeks to schedule an appointment and compete at least the first portion of testing. PMHA encourages
players to complete this testing as soon as possible as the season is in full swing and the sooner the
baseline and e-testing is completed the more complete picture the Archway Sport & Heath Therapists
have in case a player sustains a concussion. This way the therapists and doctor can build the best return
to play program option. PMHA will be tracking the completion of testing by players over the coming
weeks and months to ensure we are working towards having all players in the required age groups
completed by the end of January 2022
