**ATTENTION: Parents/Guardians
*Attention Parents/Guardians,
As some of you may or may not already know Petrolia Minor Hockey will be using the mobile app "Javelin" to handle health screening and contact tracing this year. It is easy to download and use.
To get started,
download the app for free on
Android
or
iOS
. Alternatively, you can find it on the app store or play store listed
under "Javelin". There are tutorials on the app and if you are having a
tough time downloading the app please let Nicole Veenema know.
This app has 2 parts; the first is the pre-game wellness check which needs
to be completed prior to getting to the arena by players or parents. The
second part a coach will complete the "on-field" checklist which confirms
the player has completed the first portion and that we have confirmed the
information prior to stepping on the ice.
Each Team has an invite code. If you do not know what your code is for
your team please contact Nicole Veenema @
[email protected]
for this information.
Please see the attached video which will explain how to fill in the
pre-game wellness check.
Javelin App - Setting Up Your Player(s) and Completing Wellness Check
We would like to stress the importance of this app as we navigate through
this pandemic. PMHA expects all of the players and parents/guardians who
enter the building to be registered and have the health screening app
completed before entering the arena. We understand at this time the Town of
Petrolia is also asking for the paper copy but PMHA has an obligation and a
requirement to ensure we are keeping our own tracking and tracing as well.
If the Javelin app is not used for health screening then the team could
potentially lose their ice times until they have the javelin app up and
running.
Again we appreciate your support on this,
Thank you
PMHA
