Coaches Evaluations Form 2019/20
Hello parents,
Please see below for the 2019/2020 Coaching Evaluations.
Please fill them out and submit them online using your name as any “anonymous” evaluations are not reviewed.
The intent is to supply constructive feedback on coaches that is used to help them improve and also for coaching interviews moving forward.
The scale is from 1 to 5, with the 1 being the most positive (strongly agree) and 5 the worst (strongly disagree). One good, five bad. Please be honest and respectful in evaluating your coaches.
We appreciate you taking the time to complete these as it helps us get better within the organization.
