Revised Police Check Details – Updated Link Information, News (Petrolia Minor Hockey)

To all PMHA Coaches, Bench Staff and Volunteers

Many people have inquired about how to apply for a police check. The good news is the process is completely online and can be done through the website below.

The process is as follows:

1. Contact Samantha Greaves ([email protected]) to obtain the PMHA Police check form and sign it either digitally or print and sign (it will have to be scanned back into digital form to submit)
2. Open the OPP Police Check link and follow the website directions
3. Confirmation of your police check is generally within the week and will be emailed to the address you provide.
4. Once received, please forward the document to Samantha Greaves ([email protected]) as she has taken the Police Check coordinator position once again this year.
5. Samantha will provide only the confirmed data (confirmation and dates) to the PMHA registrar to enter into the Hockey registration site allowing all volunteers to be added to the appropriate teams.
